Anchor Health Home Spotlight: Rockford Health System

Recognizing superior commitment, quality and access for people with disabilities and seniors

Spring, 2014

As we approach the end of our first year as Illinois most innovative and consumer-led health plan for people with disabilities, we are thrilled at the relationships we have built with providers that share our mission and embrace true person-centered care. So each month we want to recognize and showcase one of our provider partners that are helping to advance the state of health care for people with disabilities. This month, we want to recognize the extraordinary commitment of Rockford Memorial Health System to CCAI and the mission to serve people with disabilities and seniors in ways that support good health and help consumers live the way they want to live. RMHS embodies the CCAI model for an anchor health home with an accessible, disability-competent, consumer-centered and interdisciplinary approach to care.

Our team of case managers, nurse practitioners, and long-term services and support counselors have truly found a home at RMHS, where we work with an interdisciplinary team of medical professionals. Rockford Health System’s mission is “Superior Care. Every Day. For All Our Patients.” Through:

  • Performance Excellence
  • Innovation
  • Lifelong Learning

In the words of Gary E. Kaatz, President and Chief Executive Officer, Rockford Health System: “Our patient-centered medical home program is taking primary care to a new level. Rockford Memorial Hospital is the only area hospital to ever receive the Healthgrades Patient Safety Excellence Award and the only hospital to ever be named to the U.S. News & World Report list of Best Regional Hospitals. The list could go on and on. All of these accomplishments are because of the 3,500 physicians, employees and volunteers who take very seriously their responsibility to provide the highest level of care and compassion to patients in our region. I invite you to learn more about our organization and look forward to serving you and your family.”

We are grateful and excited to have RMHS as a key partner bringing innovation to the health care of people with disabilities and seniors.
