As we approach the end of our first year as Illinois most innovative and consumer-led health plan for people with disabilities, we are thrilled at the relationships we have built with providers that share our mission and embrace true person-centered care. So each month we want to recognize and showcase one of our provider partners that are helping to advance the state of health care for people with disabilities. This month, we want to recognize the extraordinary commitment of Rockford Memorial Health System to CCAI and the mission to serve people with disabilities and seniors in ways that support good health and help consumers live the way they want to live. RMHS embodies the CCAI model for an anchor health home with an accessible, disability-competent, consumer-centered and interdisciplinary approach to care.
