Our Unique Model of Care

Community Care Alliance of Illinois (CCAI) and Community Care Alliance of Illinois, NFP (CCAI, NFP) take a person-centered approach to care coordination. By taking the time to understand all aspects of our Members' lifestyles, we are able to promote optimal health for the whole person – body and mind.
 
As a new Member, you will receive one-on-one assistance to make your transition to CCAI or CCAI, NFP seamless. Members will have a face-to-face Care Team that works with you, providers with specialty training and a host of other services to help you get the right health care at the right time.

Anchor Health Homes

One aspect that makes CCAI and CCAI, NFP unique is our Anchor Health Homes. In addition to being fully accessible and user-friendly, you can visit an Anchor Health Home for the majority of your health care needs. This saves you the time and hassle of going to more than one doctor’s office, clinic or rehab facility. Plus, your care is better coordinated because the Anchor Health Home maintains one record containing all of your medical information.

Medical Home Education

CCAI and CCAI, NFP educates Medical Home Teams on methods to improve care capacity and capabilities to provide Wellness Programs, preventative care, management of Chronic Health Conditions and coordination and continuity of care through office visits, practitioner and provider manuals, newsletters, mailings and website updates.

Medical Home Monitoring

CCAI and CCAI, NFP will provide oversight and monitoring and support to asses Medical Home performance based on performance metrics that align with our contract with HFS and the standard and accepted PCMH criteria. Upon request by the practitioner or provider, CCAI and CCAI, NFP will provide general guidance or access to resources each individual practice may choose to utilize as part of its PCMH transformation and improvement efforts.

Crusader Community Health (FQHC)

Crusader Community Health is a community based, non-profit community health center founded in 1972 to serve the Rock River Valley area with quality health care for all people in need. It is part of a national network of Federally Qualified Health Centers serving low income and medically underserved communities. Crusader provides health care for all, regardless of their ability to pay, as it eliminates disparities in healthcare.

For 42 years, Crusader has been a major part of the Rockford, Illinois community ‘safety net,’ providing excellent medical and dental care one patient at a time. Crusader’s care reached 47,719 individuals with 205,668 visits during 2012. One in five residents trust Crusader to be their primary care medical and dental provider. More than 1,200 babies were delivered by Crusader providers at our partner hospital, that is one in four area babies.

Crusader has strategically assembled the core building blocks of a quality staff, attractive facilities and a high-tech work environment. Innovative programs such as our on-site behavioral health program and our ‘Kids Care Now’ program for no appointment pediatric care has filled an important niche in our services and community needs.

Mount Sinai gives outpatient and residential psychiatric and mental health services to adults and seniors along with:

Walk-in psychiatric crisis emergency services are on-hand 24/7 at Mount Sinai’s Emergency Department (ER). Services are geared towards you. They also are sensitive to many cultures.

SPBH gives outpatient psychiatric care for adults including:

  • Psychiatric evaluations
  • Medication management
  • Case management
  • Psychosocial rehab
  • Psychological intervention
  • Single, family, and small group therapy.
  • Programs for people of all ages who are deaf and hard of hearing are given (ASL)

Appointments are not needed. New patients are seen as “walk-ins.” Translation is available in Spanish, English and ASL.

CCAI Service AreaRockford Health Physicians, part of Rockford Health System, and formerly known as Rockford Clinic started in 1951.  Rockford Health Physicians, a multispecialty clinic, has over 170 employed physicians and over 300 physicians on staff.  At our Main Clinic, 2300 N. Rockton Avenue, there are three internal medicine primary care providers, three family practice providers, and four pediatricians along with various adult and pediatric medical specialties.  There is also a convenient care clinic located in this building.

CCAI Service AreaSchwab Rehabilitation Hospital is proud to work with Community Care Alliance of Illinois (CCAI). We offer a specialized Anchor site for people with a disability. In line with CCAI’s Model of Care, Schwab has on-site primary care providers (PCP’s). PCP’s work as a team with Schwab’s rehabilitation staff to give coordinated care. The PCP’s also work with on-site CCAI nurse care coordinators and social workers. The care they give is disability-friendly. Services that people with a disability can get at Schwab are:

  • Primary Care Provider’s (See PCP list below) who have clinics at Schwab
  • Rehabilitation health care services that involve traumatic brain injury, spinal cord injury, stroke, amputee, pediatric, musculoskeletal and pain management. General rehabilitation is also given.
  • Physical therapy

  • Speech/ language therapy
  • Inpatient acute and subacute rehabilitation
  • Orthotics and prosthetics
  • Rehab psychology services

Schwab’s Extended Services Program

Schwab‘s Extended Services Program is a source for hospital staff, patients and the local area. It gives information, disability resources and services. These services promote positive living in the community. This helps people with disabilities reach their highest level of independence and quality of life. Our disability resources can also help you and your loved ones find more information. Some topics you can learn about are advocacy, accessibility, housing and violence prevention.
