Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD).

Community Care Alliance of Illinois, NFP (CCAI, NFP) is a not for profit (NFP) organization contracted to offer Medicare benefits. The government pays companies like CCAI, NFP to provide you with the same or even more benefits than original Medicare (Part A and Part B), including affordable copay and coinsurance amounts when you use network doctors and hospitals. These are known as Medicare Advantage plans.

CCAI, NFP offers three Medicare Advantage Plans: Community Advantage (HMO), Community Complete (HMO SNP) and Community Flex-Plan (HMO-POS). Our plans include all the coverage found within original Medicare, as well as a broad range of additional benefits including:

  • Prescription drugs
  • Dental
  • Vision
  • Gym membership

Understanding the differences between original Medicare and a Medicare Advantage plan can be difficult. The Medicare.gov website is a good resource for obtaining valuable Medicare information, including benefits comparisons among the health plans in your service area.

This information is not a complete description of benefits. Contact the plan for more information. Limitations, copayments, and restrictions may apply. Benefits, premiums and/or co-payments/co-insurance may change on January 1 of each year. This information is available for free in other languages.
