Community Care Alliance of Illinois (CCAI) participates in the HFS Integrated Care Program. We cover Medicaid services through an integrated care delivery system to Seniors and Adults (aged 19 and over) with Disabilities who are eligible for Medicaid but are not eligible for Medicare. We serve Members in the counties of Boone, McHenry, Winnebago, Lake, Kane, DuPage, Cook, Will and Kankakee counties. CCAI strives to increase Members quality of care through wrap around “all-in care,” taking into account the true socioeconomic status of our Members, through a coordinated care approach.
Community Care Alliance of Illinois NFP is a contract awarded Medicare Advantage Organization offering qualifying individuals a Medicare Advantage plan (HMO) and Dual-Eligible Special Needs Plan (HMO SNP). Community Advantage (HMO) is a Medicare Advantage plan that provides Part A (Hospital), Part B (Medical) and Part D (Prescription Drug) coverage for qualified individuals. Community Complete (HMO D-SNP) covers Medicare Part A and B benefits (hospitals and medical services) while including your prescription drug coverage for those entitled to both Medicare and medical assistance from a State plan (Medicaid). Community Care Alliance of Illinois, NFP offers Medicare plans in the following counties: Cook, Kane, DuPage, Will, Boone, Winnebago, McHenry, and Ogle.
